By engaging in nutrition services with Spark Nutrition & Health, you agree to the following terms and conditions:
Confidentiality
All client records are securely stored and retained for a minimum of 10 years from the last encounter, or 10 years after the client’s 18th birthday (whichever is longer).
Clients may access their records at any time by contacting Spark Nutrition & Health.
Records may be subject to review by the College of Dietitians of Ontario if required for quality assurance, investigation, or legal purposes.
With your consent, we may communicate with other healthcare professionals involved in your care to ensure continuity of treatment.
Cancellation Policy
To avoid a cancellation fee, at least 48 hours’ notice is required to cancel or reschedule.
Less than 48 hours’ notice, or missed appointments, will result in a $45 fee.
Providing at least 24 hours’ notice allows us to offer the appointment to another client.
Payment Policy
Payment for services is due in full by the first appointment.
Accepted payment methods: e-transfer to info@sparknutritionandhealth.ca
or credit card through our secure online store.
All services are non-refundable and must be used within one year of purchase.
Insurance
Services are billed directly to the client.
A detailed receipt will be provided for submission to your insurance provider.
Please confirm coverage for Registered Dietitian services with your provider in advance.
